How to Join
The UCI Forum offers three levels of membership. Membership privileges and fees vary by participation level. View Membership Benefits.
- Adopter
- Contributing Participant
- Board Member
Membership fees are for a 12-month period, renewable on the anniversary date of the membership contract.

Adopter Level: $5,000
To join the UCI Forum as an Adopter, you will need to
- Complete the Adopter Membership Agreement in full and have signed by an authorized employee of your company
- Complete the Member Contact Form
- Email, fax or send both the completed and signed membership agreement and the registration form to the address below
Contributing Participant Level: $25,000
To join the UCI Forum as a Contributing Participant, you will need to
- Complete the Contributing Participant Membership Agreement in full and have signed by an authorized employee of your company
- Complete the Member Contact Form
- Email, fax or send both the completed and signed membership agreement and the registration form to the address below
Board Member Level: $75,000
